Our commitment
Nu Life Health and Wellness is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
Treatment. We use your health information to provide, coordinate, and manage your care, including sharing it with other providers involved in your treatment — laboratories, pharmacies, specialists, and hospitals.
Payment. We may use and disclose your health information to bill and collect payment for the care we provide, and to verify coverage where applicable.
Health care operations. We may use your health information for internal operations such as quality assessment, staff review, training, and business planning.
Appointment reminders and follow-up. We may contact you to remind you of appointments or to follow up on your care, using the contact details you provide.
As required by law. We may disclose your health information when required by federal, state, or local law, including for public health activities, reporting of abuse or neglect, health oversight, judicial proceedings, and law enforcement purposes as permitted by law.
Uses that require your written authorization
Most uses and disclosures of psychotherapy notes, uses for marketing purposes, and any sale of protected health information require your written authorization. Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time in writing.
Your rights regarding your health information
- Right to inspect and copy. You may inspect and obtain a copy of your health information, in electronic form where we maintain it electronically.
- Right to amend. You may request an amendment if you believe information in your record is incorrect or incomplete.
- Right to an accounting of disclosures. You may request a list of certain disclosures we have made of your health information.
- Right to request restrictions. You may request restrictions on how we use or disclose your information. We are not required to agree to all requests, but we must agree to a request to restrict disclosure to a health plan for a service you paid for in full out of pocket.
- Right to confidential communications. You may request that we contact you at a particular address or by a particular method.
- Right to a paper copy. You may request a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Right to be notified of a breach. We will notify you if a breach occurs that may have compromised the privacy or security of your information.
Website forms are not a secure channel for health information
The contact and newsletter forms on this website are handled by third-party service providers that are not covered by a business associate agreement. Please do not submit protected health information through them. To discuss anything medical, call the clinic at (813) 492-4420.
Changes to this notice
We reserve the right to change this notice and to make the revised notice effective for health information we already hold as well as information we receive in the future. The current notice will always be posted on this page with its effective date.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our practice using the contact details below, or with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
